What is calcific tendinopathy?

Calcific tendinopathy, sometimes called calcific tendinitis, is a condition where a deposit of calcium builds up within a tendon, most commonly in the rotator cuff of the shoulder (especially the supraspinatus tendon). The deposit can irritate the tendon and surrounding tissues, causing pain and restricting movement.

The exact cause is not fully understood, but it is thought to involve changes in the tendon’s blood supply and cell activity that lead calcium to accumulate. It most often affects adults aged 30 to 60 and is more common in women.

Calcific tendinopathy can sometimes resolve on its own as the body slowly reabsorbs the calcium, but this can take a long time and be very painful, which is why many people seek treatment such as barbotage to break up and remove the deposit far sooner.

What are the symptoms, and who does it affect?

The dominant symptom is shoulder pain, which can range from a dull ache to sudden, severe pain when a deposit is in its most inflamed phase. Common day-to-day signs include:

  • Pain when raising the arm, reaching overhead or lying on the affected side.
  • Pain that is often worse at night and can disturb sleep.
  • Stiffness and reduced range of movement in the shoulder.
  • A sudden flare of intense pain when a large deposit becomes acutely inflamed.

It most commonly affects adults aged 30 to 60, is more frequent in women, and has been associated with conditions such as diabetes and thyroid disorders. Some people have a calcium deposit visible on imaging with few or no symptoms, while others have severe pain. An ultrasound assessment confirms the diagnosis and rules out other causes.

How is it diagnosed, and how is the calcium deposit located?

At the Harley Street Ultrasound Group, your appointment begins with a diagnostic ultrasound assessment, performed by your specialist consultant doctor. Ultrasound shows the tendons and any calcium deposit in real time, pinpointing its exact size and position, and is used to confirm calcific tendinopathy and, just as importantly, to rule out other causes of shoulder pain such as a rotator cuff tear, frozen shoulder or bursitis, which can feel similar but need different treatment.

Locating the deposit precisely is what makes barbotage possible. Because the same ultrasound is then used to guide the injection, your consultant can place the needle exactly into the target structure, rather than by external approximations.

If the assessment shows that a different treatment would serve you better, your consultant will tell you on the day. See “What if I don’t need an injection?” below.

What is barbotage and how does it work?

Barbotage is an ultrasound-guided procedure that breaks up and removes a calcium deposit from the tendon. Also known as ultrasound-guided needle barbotage or lavage, it is the leading non-surgical treatment for painful calcific tendinopathy.

Under real-time ultrasound, your consultant guides a fine needle directly into the calcium deposit, something that is only possible because the imaging shows exactly where the deposit lies. Sterile fluid is injected to soften the calcium, which is then broken up and drawn back out (aspirated) through the needle. A small steroid injection into the subacromial bursa often follows to settle the inflammation and ease pain.

Because the whole procedure is performed under live imaging, the needle is confirmed in the deposit rather than estimated by feel, and nearby nerves and blood vessels are identified and avoided. This is what makes barbotage both effective and a safe alternative to surgery for most patients.

Does barbotage hurt? What should I expect on the day?

Most patients find barbotage far more comfortable than expected. Local anaesthetic is used, so you typically feel a small sting as it goes in, followed by a sensation of pressure as the deposit is worked on. This is normal and settles quickly.

The appointment usually takes around 20–40 minutes from assessment to completion:

  • Your consultant scans the shoulder to confirm the diagnosis and locate the deposit.
  • The skin is cleaned and local anaesthetic is applied.
  • The needle is guided into the calcium under live ultrasound, and the deposit is broken up and aspirated.
  • A steroid injection may be given into the bursa, then you rest briefly and receive aftercare guidance before you leave.

There is no general anaesthetic and no overnight stay. You can usually drive yourself home, though arranging a lift is sensible if your shoulder feels sore afterwards.

Who will perform my barbotage?

Your treatment is performed by a consultant specialist doctor, not a physiotherapist, nurse or GP. This matters, as the majority of MSK injections in the UK are delivered by non-consultant practitioners, often without ultrasound image guidance.

Your consultant assesses the shoulder, confirms the source of pain, performs the procedure and explains your aftercare personally.

What happens if I don’t need an injection?

We will never give you an injection you do not need. If, after your ultrasound assessment, your consultant decides barbotage is not the right treatment for you, the procedure simply won’t go ahead. You will be charged at our standard diagnostic ultrasound rate, not the full treatment price.

This is a deliberate commitment to clinical integrity. Some providers treat every appointment as a guaranteed procedure; we don’t. You can book knowing that you will receive a genuine specialist assessment first, and that treatment only proceeds when a senior clinician believes it is genuinely warranted.

If an injection isn’t appropriate, your consultant will explain why and set out the alternatives. This may include physiotherapy, a different injection, such as PRP, or onward referral, meaning you still leave with a clear plan.

How soon will I feel better, and what is the success rate?

Many patients notice a meaningful reduction in pain within a few weeks, as the inflammation settles and the tendon recovers. It is normal to have a short period of soreness in the first day or two as the area calms down, after which comfort and movement steadily improve.

Barbotage has a good success rate for relieving pain and improving shoulder function, and for many people a single treatment is enough to allow the body to clear any remaining calcium over time. Results vary with the size and nature of the deposit and individual factors; some patients benefit from a repeat procedure or a course of physiotherapy. Your consultant will give you a realistic picture based on your own assessment.

What is the recovery and aftercare?

There is no significant downtime. You can return to everyday activities the same day, though it is sensible to avoid heavy lifting or strenuous shoulder use for a few days. Some mild soreness or a feeling of fullness in the shoulder for a day or two is normal, and applying an ice pack can help.

Once the initial soreness settles, gentle, regular movement and stretching help restore full shoulder function. Your consultant will recommend specific exercises, and many patients benefit from a short course of physiotherapy alongside the treatment.

How does barbotage compare with a steroid injection, shockwave or surgery?

There are several ways to treat calcific tendinopathy, and the right choice depends on your symptoms and the deposit:

  • Barbotage directly breaks up and removes the calcium deposit under ultrasound, addressing the root cause. Often the most effective option for a painful, well-defined deposit.
  • A steroid injection alone can ease pain and inflammation but does not remove the calcium, so symptoms may return. It is sometimes used first, or alongside barbotage into the bursa.
  • Shockwave therapy uses sound waves to help break down the deposit over several sessions; non-invasive but typically slower and not always available.
  • Surgery is reserved for the minority of cases that do not respond to less invasive treatment.

Barbotage is generally preferred where a clear calcium deposit is the main problem, because it tackles the deposit itself with a single, minimally invasive procedure, treating calcific tendinopathy effectively without surgery for most patients. Your consultant will recommend the most appropriate option after your assessment.

How much does barbotage cost in London?

The treatment I was given was excellent by Dr. Ahmed who explained everything thoroughly and ensured that I understood the procedure before going ahead with an injection in my arm.
