Ultrasound-Guided Cortisone Injections

An ultrasound-guided steroid (cortisone) injection in the ankle is used to relieve pain and reduce inflammation from a range of ankle conditions. The most common are:-

• Ankle osteoarthritis: wear of the joint cartilage, often years after a previous fracture or sprain, causing deep joint pain and stiffness.
• Tendon-related pain (tendinopathy): inflammation around the tendons that run across the ankle, such as the peroneal or tibialis posterior tendons.
• Ankle impingement: pinching of soft tissue or bone at the front or back of the ankle, often in athletes and former dancers.

It can also help inflammatory arthritis affecting the ankle, and persistent pain after an injury that has not settled with rest, footwear changes or physiotherapy. An ultrasound assessment confirms which structure is responsible before any injection is given.

An ankle injection is typically considered when ankle joint pain is interfering with walking, standing, sport or work, and simpler measures have not given lasting relief. Common signs include:

• A deep ache in the joint that is worse on weight-bearing or first thing in the morning.
• Swelling, stiffness or a reduced range of movement at the ankle.
• Pain localised to a tendon when you push off, turn the foot or climb stairs.

It is most suitable for people with a clearly localised, inflammatory source of pain that ultrasound can identify, for example a single arthritic joint or an inflamed tendon sheath. Your consultant will confirm whether an injection is the right next step for you, or whether another treatment would serve you better.

Diagnosing the problem accurately first is what makes the treatment precise. Because the same ultrasound is then used to guide the injection, your consultant can place the needle exactly within the ankle joint or tendon sheath under real-time, rather than by external lapproximations, identifying and avoiding nearby nerves and blood vessels.

An ultrasound-guided steroid injection treats ankle pain by delivering cortisone directly into the affected joint or tendon sheath, where it reduces the inflammation that drives the pain. Cortisone is a powerful anti-inflammatory; placing it precisely at the source calms the swelling and allows the joint or tendon to move more comfortably.

Because the procedure is performed under real-time ultrasound, your consultant can ensure the injection reaches the exact location and confirm the medication is going precisely to where it is needed. This precision is what makes the treatment both effective and safe, avoiding nearby nerves and blood vessels.

The injection is often combined with local anaesthetic, which both eases the procedure and helps confirm the source of pain. It is most effective when paired with the right footwear, activity changes and, where needed, physiotherapy, your consultant will set out the full plan.

Most patients find an ankle injection far more comfortable than expected. Local anaesthetic is used, so you typically feel a small sting as it goes in, followed by brief pressure as the medication is delivered. This is normal and settles quickly. Using ultrasound guidance also means fewer attempts are needed to reach the target.

The appointment usually takes around 20–30 minutes from assessment to completion:

• Your consultant scans the ankle to confirm the source of pain.
• The skin is cleaned and local anaesthetic is applied.
• The injection is delivered under live ultrasound guidance.
• You rest briefly and are given aftercare guidance before you leave.

There is no general anaesthetic and no overnight stay. Side effects are uncommon and usually minor, a short-lived flare of pain in the first day or two, or temporary skin changes at the site. Your consultant will talk you through these before you proceed.

Your injection is performed by a consultant specialist doctor, not a physiotherapist, nurse or GP. This matters, as the majority of MSK injections in the UK are delivered by non-consultant practitioners, often without ultrasound image guidance.

This is a deliberate commitment to clinical integrity. Some providers treat every appointment as a guaranteed procedure; we don’t. If an injection isn’t appropriate, your consultant will explain why and set out the alternatives. This may include physiotherapy, a different injection, such as PRP, or onward referral, meaning you still leave with a clear plan.

Many patients notice improved comfort within a few days to two weeks, as the cortisone takes effect. The local anaesthetic may give a few hours of immediate relief on the day, which then wears off before the steroid’s anti-inflammatory effect builds over the following days.

How long the relief lasts varies. For some people a single ankle injection settles the problem for several months or longer; for others it provides a valuable window to progress rehabilitation and footwear changes. Results depend on the underlying condition. Arthritis, tendon problems and impingement each respond differently, and your consultant will give you a realistic picture based on your own assessment. Where helpful, the injection can be repeated within safe limits.

There is no significant downtime. You can usually walk out and return to light daily activities the same day. We recommend a short period of relative rest, avoiding high-impact activity such as running or prolonged standing for around 48 hours to let the cortisone settle and to protect the joint or tendon while inflammation reduces.

Sensible aftercare makes a real difference to how long the benefit lasts:

• Wear supportive, well-cushioned footwear and avoid unsupportive flat shoes for a while.
• Reintroduce activity gradually, building back up over a week or two.
• Follow any stretching or strengthening exercises your consultant recommends, often alongside physiotherapy.

A steroid injection is usually offered as part of a step-by-step approach. Most ankle problems are first managed with activity changes, supportive footwear or orthotics, pain relief and physiotherapy. An ultrasound-guided cortisone injection is the next step when those measures have not given enough relief, or to reduce inflammation so rehabilitation can progress.

Other options your consultant may discuss include a course of physiotherapy, bracing, or different injectable treatments depending on the diagnosis. The advantage of the injection is that it is targeted, low-risk and recovery is quick, with no general anaesthetic.

Surgery is generally considered only later, for advanced ankle arthritis or structural problems that have not responded to non-surgical care. For many patients, a precisely placed injection provides meaningful relief and helps delay or avoid more invasive treatment.
